HRT

Can dose of oestrogen or progesterone affect bleeding pattern? I have a 53 year old on E2 only patch plus utrogestan 100mgs nightly, for 2 years, and now having monthly bleed.Is it reasonable to increase utrogestan to 2 tablets every night and try this for 3 months or would you carry out PV in the first instance? Recent smear normal.

women who have bleeding on HRT, if you refer them for a scan, you will be advised to tell the patient to stop HRT for 6 weeks first and or refer to gynae but most women do not want to stop HRT. What would you advise ?

Note for GPs in the UK: This event is not formally accredited by an external organisation for UK CPD points. The current guidance for GP CPD in the UK is that it is appropriate that the credits you self-allocate should equal however many hours you spent on learning activities, as long as they are demonstrated by a reflective note on lessons learned and any changes made or planned (if applicable).
