The transition from paediatric dental services to special care dentistry can be a daunting process for young adults, especially those with complex medical needs or disabilities. This transition is often hindered by barriers such as a lack of available dental professionals, patients' reluctance to leave familiar care providers, and in some situations the inability to self-advocate at dental visits. This webinar aims to explore these challenges and to discuss the formalised care pathway developed in Lancashire Teaching Hospitals NHS Foundation Trust (LTHTR) to ensure a smooth and supportive transition for these vulnerable patients.
