Description

This is the 4th and final part of Dr. Grauer's initial ECG Interpretation Series. The focus of this series has been on introducing and applying Dr. Grauer's user-friendly Ps,Qs,3R Approach for systematic rhythm interpretation ( = "Watch your Ps, Qs & 3Rs").

The first 2 sessions in this series looked at applying the Ps,Qs,3Rs to wide tachycardias. Session 3 distinguished between Mobitz I vs Mobitz II 2nd-degree AV block. Today's 4th session advances assessment of the AV blocks to the concepts of AV Dissociation vs "high-grade" vs Complete ( = 3rd-degree) AV block.

Future ECG series by Dr. Grauer will present and apply a systematic approach to 12-lead ECG interpretation β€” covering key issues including recognizing infarction due to acute coronary occlusion when ST segments are not elevated ( = the concept of "OMI" rather than "STEMI" ) β€” as well as standard topics such as chamber enlargement, bundle branch blogs and MUCH more.

Learning objectives

  1. To review the criteria for the ECG diagnosis of Complete ( = 3rd-Degree) AV Block.
  2. To illustrate the 3 Types of AV Dissociation (ie, by "default" β€” "usurpation" β€” or AV block)
  3. To apply the above for determining when AV Dissociation is Complete AV Block. =========================== KEY CONTENT NOTES: β€” My "system" for time-efficient Rhythm Interpretation = "Watch your Ps, Qs & 3Rs ( = P waves or atrial activity β€” QRS duration β€” Rate/Regularity/"Related") ====================== "KISS" Method for the AV Blocks (There are 3 Degrees of AV Block). β€” 1st Degree = is EASY ( = sinus rhythm + long PR). β€” 3rd Degree = is also usually EASY (because the ventricular "escape" rhythm will usually be regular or almost regular β€” P waves "march thru"). β€” So if neither 1st nor 3rd degree AV block β€” it is 2nd-degree (of which there are 3 Types) = Mobitz I β€” Mobitz II β€” 2nd degree with 2:1 conduction (which could be either Mobitz I or Mobitz II). β€” "Other Stuff" (AV dissociation β€” "high-grade" block β€” AV block "mimics" (ie, like blocked PACs).
