The reason why it is not flat T waves as it is hypokalemia, and the question asks what is the least likely to be seen- hence the answer is Delta waves, which is seen in syndromes like WPW.
One thing to note is that this is a negative question (leask like), and is not usually indicative of the questions in the real exam, however it is still a good tool of learning.
