Baseline Risk and Toxicity Education: Outline expected adverse event (AE) profiles for targeted agents (e.g., third-generation ALK inhibitors like lorlatinib), including central nervous system (CNS)/neurocognitive effects, hyperlipidemia, hepatic transaminase elevations, and peripheral edema or neuropathy.
Structuring Dosing and Tolerability Strategies: Acknowledge that starting at lower dose levels (or utilizing dose reductions) can preserve efficacy while significantly lowering toxicity rates for susceptible patients.
Comprehensive Readiness Assessment: Evaluate patient readiness across five key domains before initiating therapy:

Identifying Subtle Neurocognitive and Functional Changes:

  • Screen routinely for subtle early signs: forgetfulness, word-finding difficulty, reduced attention span, altered sleep patterns, irritability, apathy, or mood swings.
  • Track functional indicators such as task completion delays, work errors, or needing new reminders for daily activities.
  • Leverage care partner observations, as family members frequently notice subtle personality or behavioral shifts before the patient recognizes them.
    Rapid Clinical Triage and Workup:
  • Perform rapid bedside or remote functional screens (orientation, counting backwards, item recall, medication management capability).
  • Acute Escalation Triggers: Immediate evaluation in an urgent or emergency setting is required for acute confusion, hallucinations, seizures, focal neurological deficits, or sudden loss of independence.
  • Rule out medical or oncologic emergencies (e.g., stroke, disease progression, hypoglycemia, acute drug-drug interactions) via laboratory testing, blood glucose checks, and brain MRI prior to attributing symptoms solely to drug toxicity.
    Actionable AE Mitigation Protocols:
  • Mild Toxicity / Non-Functional Impact: Continue therapy with close monitoring or supportive strategies (e.g., lifestyle modifications, lipid-lowering agents).
  • Moderate-to-Severe Toxicity / Functional Impairment: Implement temporary treatment holds to allow recovery to baseline, followed by dose reductions. Reassess within one month of any modification to confirm symptom resolution.

3. Active Patient and Care Partner Participation in Symptom Monitoring

Low-Tech Communication Tools: For patients with limited digital access or transportation barriers, provide simple, low-tech solutions:

  • A bilingual one-page start-up checklist detailing dosing instructions, scheduled lab checks, and emergency contacts.
  • A symptom diary utilizing simple scales (e.g., 0–3) or prompts focused on memory, sleep, mood, and fall history.
    Structured Caller Scripts: Educate care partners to report changes using clear, concrete communication frameworks (e.g., "T_oday [Patient] did [specific action], and now exhibits [specific symptom_]") to assist triage personnel.
    Immediate Home Safety Protocols: Instruct caregivers on actionable home safety steps when neurological changes occur, including stopping driving immediately, securing hazardous appliances, utilizing pill organizers, and keeping emergency numbers accessible.

4. Workflow Integration & Preference-Informed Decision-Making

Clear Reporting Pathways: Establish explicit symptom escalation thresholds on printed patient cards:

  • Immediate ("Call Now / Red Flags"): New or severe confusion, behavioral changes, seizures, loss of consciousness, jaundice, or fever with abdominal pain.
  • Graded ("Call Soon"): Progressive neuropathy impacting daily activities, persistent nausea/vomiting, or new palpitations/syncope.
    Multidisciplinary Workflow Alignment:
  • Utilize scripted nurse triage to conduct standardized symptom assessments upon patient contact.
  • Schedule proactive clinical touchpoints (e.g., nurse phone check-ins at Day 3 and Week 2).
  • Integrate multidisciplinary specialty teams early—including oncology, pharmacy (for drug-drug interaction screening), social work (for transportation, language, and financial barriers), neurology, and hepatology.
