- High BMI / Low Inflammation: Symptoms may be mistakenly attributed to uncontrolled peripheral arthritis, potentially leading to unnecessary escalation of biologic therapy while the underlying metabolic trigger is overlooked.
- High BMI / High Inflammation (Highest-Risk Zone): There is an ambiguous and mixed signal here, where active synovitis and obesity-related inflammation coexist. An automatic switch to biologic therapy should be avoided until the effects of both factors have been carefully assessed.
- Low BMI / Low Inflammation: Composite scores are highly reliable and reflect true, well-controlled disease activity.
- Low BMI / High Inflammation: Confirms to the physician that the strong inflammatory signal primarily originates from joint or synovial structures, justifying optimization of immunomodulation.

Supplement to Total Scores: Because obesity artificially inflates patient tenderness assessments, composite scores (e.g., DAPSA) can be misleading. Objective markers, such as CRP levels and advanced imaging (ultrasound or MRI), should be integrated to distinguish active synovitis from weight-bearing mechanical joint stress.
Individual Treatment Strategies and Latest Findings from Clinical Trials

Impact of Weight Loss on Minimal Disease Activity: Successful weight loss is directly linked to achieving therapeutic goals. Clinical data show that the probability of achieving such an outcome increases significantly with the extent of weight loss:
- <5% Weight Loss: 23.1% success rate.
- 5%–10% Weight Loss: 44.8% success rate.
- >10% Weight Loss: 59.5% success rate (Odds Ratio: 6.67).

The TOGETHER-PsA Phase 3b Study: This 52-week randomized study examined 271 overweight/obese adults with highly active PsA, comparing ixekizumab (an IL-17A inhibitor) in combination with tirzepatide (a dual GLP-1/GIP receptor agonist) against ixekizumab monotherapy.

Step 3 (Decision): If there is no active synovitis and metabolic optimization has not yet been sufficiently attempted, escalation of immunomodulatory therapy should be postponed. Instead, the focus should be on weight loss, and the joints should be re-evaluated in 3–6 months.

Stigmatization and Patient Perspective: The topic of weight is emotionally sensitive; many patients report significant burdens or frustrations due to previous doctor visits. Physicians should avoid presenting obesity as a personal failure regarding lifestyle or discipline and instead define it as a bidirectional biological disease modifier.

Key Communication Principles:
- Use Collaborative Openings: Always obtain express consent before introducing the topic of body weight into the conversation.
- Focus on Metabolic and Joint Health: Explicitly link weight reduction to the relief of symptoms, the reduction of systemic inflammatory load, and the lowering of mortality due to cardiovascular risk factors.
- Practical Recommendation on Timing: If a patient is highly symptomatic, in severe pain, or in emotional distress during an initial visit, focus exclusively on the immediate stabilization of joints and skin first. Postpone detailed discussions about metabolism and obesity treatment to the second or a later appointment, once a therapeutic rapport has been established.

- Use Phrasing Like: "Is it okay if we talk about how your metabolic health relates to your PsA?" or "Some of the causes of your symptoms may be due to inflammation related to your body weight. Can we explore that together?"
Coordinated Care Pathways and Integration of Systemic Obesity Therapy

Strategic Classification by BMI Thresholds: Care coordination processes are systematically aligned with the severity of the patient's metabolic profile:
- BMI 30–34: Primarily rheumatology-led. The focus is on optimizing immunomodulatory therapies in conjunction with basic nutrition, diet, and exercise counseling.
- BMI 35–39: Transition to a shared care model. The rheumatologist initiates discussions about metabolism and monitors progress while actively collaborating with primary care physicians or endocrinologists to monitor and optimize obesity therapies, blood pressure, and early blood sugar control.
- BMI > 40: Requires formal referral to a multidisciplinary team (MDT). The rheumatologist ensures adequate treatment of peripheral joints and potentially the spine, while specialists in obesity medicine or bariatric surgeons should lead advanced weight loss strategies.

Handling Modern Obesity Therapies (GLP-1/GIP Receptor Agonists):
- Titration During Adverse Events: While overall discontinuation rates in studies due to gastrointestinal events are low (~3%), 20-30% of patients experience transient side effects such as nausea, diarrhea, constipation, or delayed gastric emptying. Dosing must therefore be increased gradually to improve long-term tolerability for the patient.
- Collaboration Infrastructure: Physicians should utilize resources available in the clinic – such as pharmacy staff, nurse practitioners, and patient support programs – to handle the paperwork for prior authorizations, implement systematic titration schedules, and monitor patient tolerability in everyday life.
