Evolving Paradigms in Lung Cancer

Welcome to our new #accredited #SocialMediaLearningSeries on Evolving Paradigms in Lung Cancer, part of our ongoing micro-learning series on Clinical Integration of PD−(L)1×VEGF Bispecific Therapy.

Lung cancer care is evolving — are you ready for what’s next?

New treatment strategies, emerging evidence, biomarkers, and evolving approaches to safety are reshaping clinical decision-making.

Across this social learning series, explore the why, what, and how behind these advances through bite-sized expert insights designed to help you connect emerging evidence with everyday practice.

PD-(L)1 x VEGF bispecifics: What changes?

Checkpoint inhibition transformed lung cancer care - but immune suppression and tumor angiogenesis remain interconnected barriers.
PD-(L)1 blockade -> restores antitumor T-cell activity
VEGF inhibition -> targets angiogenesis and may modify the immunosuppressive tumor microenvironment
PD-(L)1 x VEGF bispecifics -> integrate both targets within a single molecule
The shift is not simply two targets instead of one - it is an attempt to coordinate immune and vascular modulation within the tumor microenvironment.

PD-(L)1 × VEGF bispecifics: Watch the emerging evidence

Clinical development is accelerating across lung cancer.
In HARMONi-2, ivonescimab improved median PFS versus pembrolizumab in first-line, PD-L1-positive, driver-negative advanced NSCLC:

  • 11.14 vs 5.82 months
  • HR 0.51; P<0.0001
Beyond HARMONi-2, emerging data from HARMONi-A, HARMONi-6 and the ROSETTA programme are expanding the evidence across EGFR-mutant, squamous and non-squamous NSCLC, with development also extending into SCLC.
The question is no longer only “does dual targeting make biological sense?” — but where might these efficacy signals translate into clinical value?
    Investigational / emerging clinical evidence.

Which patient? Think beyond a single biomarker

As PD-(L)1 x VEGF bispecific evidence evolves, patient selection will require more than one number.
Molecular profile - actionable drivers?
PD-L1 expression - what does the available dataset show across subgroups?
Histology - squamous or non-squamous?
Disease behavior - indolent or rapidly progressing?
Clinical risk - does the patient's baseline profile alter the benefit-risk calculation?
Patient selection = clinical context + tumor biology + emerging evidence.

Dual-pathway therapy requires dual-domain vigilance

PD-(L)1 x VEGF targeting brings two familiar toxicity domains into the same conversation.
Before treatment: assess blood pressure, renal function/urinalysis, relevant comorbidities, and baseline risk.
During treatment: track blood pressure and proteinuria/renal function; remain alert for bleeding and immune-related adverse events across organ systems.
Escalate early: involve the appropriate multidisciplinary team before a manageable toxicity becomes treatment-limiting.
Reactive management starts after toxicity appears. Proactive management starts before it does.
